 | Molecular Biology of human cancers Book Description Over the last three decades, knowledge on the molecular biology of human cancers has vastly expanded. A host of genes and proteins involved in cancer development and progression have been defined and many mechanisms at the molecular, cellular and even tissue level have been, at least partly, elucidated. Insights have also been gained into the molecular mechanisms underlying carcinogenesis by chemical, physical, and biological agents and into inherited susceptibility to cancer. Accordingly, Part I of the book presents many of the molecules and mechanisms generally important in human cancers. Following an overview on the cancer problem, individual chapters deal with cancer genetics and epigenetics, DNA damage and repair, oncogenes, tumor suppressors, regulatory pathways in cancer, apoptosis, cellular senescence, tumor invasion, and metastasis. A consensus is emerging that while these common mechanisms and molecules are all relevant to human cancers, in each cancer type (or even subtype) a selection of them are extremely important. For selected cancers, the route from genetic and epigenetic changes to their biological and clinical behavior can already be traced. Part II of the book presents a broad, but exemplary selection of cancers that serve as paradigms to illustrate this point. In fact, cancer research has now reached a critical stage, in which the accumulated knowledge on molecular mechanisms is gradually translated into improved prevention, diagnosis, and treatment. The state, pitfalls, and potential of these efforts are summarized in Part III. More than ever, cancer research is now an interdisciplinary effort which requires a basic knowledge of commonly used terms, facts, issues, and concepts.  | Cancer Immune Therapy :Current and Future Strategies Editorial Reviews From the New England Journal of Medicine, The dream of curing cancer by exploiting natural immunity goes back at least as far as the time of Paul Ehrlich, when the potential of the immune system was only a subject of speculation. Since then, having made substantial progress in understanding the molecular and cellular aspects of the immune system, we have begun to fulfill this dream -- at least for some patients. This book is not, however, about these clinical advances. Rather, it selectively explores emerging cellular and therapeutic approaches that may contribute to clinical advances over the next two decades. However, because the selection of topics is so random, the book lacks a predominant leitmotif that advances a specific hypothesis or concept. Investigational approaches to the identification of human tumor antigens with the use of cultured T cells or serologic analysis by recombinant expression cloning are described, but the analysis of genes and proteins and T-cell-receptor spectrotyping for this purpose are overlooked. Dendritic-cell vaccines are described, but peptide, anti-idiotypic, and allogeneic-cell vaccines are not. One of the early strategies used in clinical immunotherapy, bacterial vaccination, is well reviewed in the modern context of natural immunity through toll-like receptors. Interferons, also mediators of natural immunity, are mentioned in a good chapter on the regulation of the major histocompatibility complex. But no chapter covers the role of interferons in influencing antigen processing or immune effector-cell function or their therapeutic activity in both solid tumors and hematologic cancers. Although one whole chapter and part of another are devoted to interleukin-10, other cytokines (such as interleukin-2, interleukin-4, interleukin-12, and interleukin-18) merit only slight mention in the very complete and erudite chapters on transforming growth factor (beta) as an immunosuppressor and in a chapter on cytokines fused to antibodies. A chapter is devoted to preclinical investigations of monoclonal antibodies conjugated to chemical toxins, but there is no discussion of the clinically approved toxin ozogamicin conjugated to gemtuzumab. Other topics not included are the mechanism of the clinically effective engineered monoclonal antibodies (rituximab, ibritumomab, and alemtuzumab), two of which, rituximab and ibritumomab, are effective both alone and as radioimmunoconjugates. The editors of this monograph state that their goal is to review the "current understanding of tumor immunology with special emphasis on the inter-relationship of the tumor and immune system, and cancer immune therapy." A further understanding of the scientific basis of tumor immunology without a comprehensive clinical review is a worthwhile objective. But lessons can also be derived from a review of clinically effective approaches that may or may not yet have reached their full potential. On both counts, and because there are so many gaps in the selection of the subjects covered, this book should be valued mainly for the understanding it provides to readers interested in the individual topics. In many ways it is an update and extension of Bearison and Mulhern's textbook, Pediatric Psychooncology (New York: Oxford University Press, 1994), in that it covers many of the same topics with some of the same authors. However, the contributors to the new book represent several nations, primarily Israel, whereas the Bearison and Mulhern book is written strictly by U.S. authors. As a result, the considerable overlap among the chapters allows the reader to recognize interesting international differences in thought and practice in regard to meeting the psychosocial needs of children with cancer and their families. For example, a chapter by a U.S. author states that more than half of children with progressive cancer die at home, whereas an Israeli contribution indicates that only rarely does the death of the child occur at home. Also, some authors leave it to the parents to decide whether or not the child should be told the diagnosis and prognosis. For me, the critical need for honesty with children was confirmed 40 years ago by Vernick and Karon in their landmark paper, "Who's Afraid of Death on a Leukemia Ward?" (American Journal of Diseases of Children 1965;109:393). Currently, in the United States, clinicians need to obtain informed consent of children older than seven years of age to register them in cancer studies, which leaves no option but candor. This entails patient, tactful discussion among the health care team, the child, and the family so that all can communicate openly. This book contains many outstanding discussions that are valuable to all who care for children with cancer. The chapters by Wolfe et al. on palliative care, Hain on pain control, and Postovsky and Ben Arush on "do not attempt to resuscitate" offer especially practical and useful information. Highly illuminating to me were the chapters on the role of fantasy, art therapies, humor, and pets and on complementary and alternative therapies. The lengthy, well-organized, detailed review of the measurement of the quality of life in children deserves to become a standard reference. Most representative of quantitative science is the chapter by Mulhern and colleagues on the neuropsychological sequelae of childhood cancer and its treatment. This chapter is based on a review of the literature and the extensive clinical studies that have been conducted at St. Jude Children's Research Hospital since 1970 -- the year we recognized that acute lymphoblastic leukemia was no longer an incurable disease. Some general statements in the introduction need to be modified. The first sentence says that cancer is the leading cause of death from disease in children past the neonatal period.  | Oncogenomics: Molecular Approaches to Cance Cancer is not a single genetic disease but, rather, hundreds of diseases consisting of various combinations of genetic alterations. Many types of genetic alterations contribute to neoplastic transformation. The evidence for this statement has now become common knowledge. In a similar vein, almost everybody believes that genome sequencing is paving the way to a revolution in biology and medicine in general but in oncology in particular. We all mention imatinib and a few monoclonal antibodies when we want to demonstrate that targeted therapies are already more than just a vision. We are, however, still unable to foresee clearly the role genomics will play in the prevention, diagnosis, and treatment of cancer in the next 10 or 15 years. This uncertainty stems mainly from a dearth of clinical data, and most of the data we do have are retrospective and therefore most probably biased. As stated in its introduction, this book is probably the first of its kind devoted to the genomics of cancer. I approached it with mixed feelings: on the one hand, I was hoping to find answers to some of my questions, mainly because the span of the book reaches from molecular profiling to model systems for discovering and validating drug targets, and from molecularly targeted pharmacology to clinomics. The latter is a term coined by Daniel Von Hoff et al., whose chapter in this book defines clinomics as the application of genomics to patient care. The essence of the book is to bring together scientists from various specialties and share the same language, a prerequisite for the next "leap forward."
